POSITION SUMMARY
Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination and diagnostic tests. Interprets the data, develops and implements therapeutic plans.
This position provides clinical support, education, and staff development, case management and supervisory functions for pain & symptom management, in the home, hospital and extended care facilities settings. It also includes educating/mentoring nurses and other clinicians in evidenced-based pain & symptom management, assisting with post hospital discharge care planning.
The Nurse Practitioner in Hospice & Palliative Care provides leadership in ensuring excellent patient care services, coordinating educational and outreach services, leading quality improvement initiatives, and advocating for Best Practices in symptom management and quality end of life care.
The Nurse Practitioner in Hospice and Palliative Care provides consultation, utilizing expert knowledge and competencies from a foundation of evidence-based practice. This position will proactively identify opportunities to improve the patient and family experience of coping with and managing pain and symptoms related to advancing disease and progression to end of life. This Nurse Practitioner position will provide leadership in the development of, implementation of, and use of measures that ensure team effectiveness in improving patient quality, service, and efficient resource use.

1. Possess a Masters Degree in nursing or a related field.
2. Minimum of five years direct patient care experience required.
3. Experience in staff development, education, or precepting nurses and other clinicians.
4. Practical experience in internal medicine, geriatrics, or oncology preferred (experience appropriate to complex case management with issues of aging).
5. Demonstrates skills in clinical quality improvement and in resource management.
